# Choose questions and templates

> Build a clear rating and open-feedback questionnaire without unnecessary duplication.

The quality of analysis depends on the clarity and structure of the questionnaire.

## Use templates intentionally

Templates provide a practical starting point. Review every included item instead of assuming the whole template fits your context.

## Add questions from the bank

Question-bank items use canonical wording and categories. Reusing them improves consistency and may support clearer comparisons than custom variations.

## Add custom questions

Custom questions are useful for company-specific priorities. Keep each question focused on one idea and avoid combining two experiences in one statement.

## Include open feedback

Open-text questions explain why a score moved and surface issues you did not anticipate. Use a small number of purposeful prompts so participants know what kind of detail is useful.

## Review the final canvas

* Remove duplicate or near-duplicate questions.
* Group related topics logically.
* Put sensitive questions after participants understand the survey.
* Check that rating direction is consistent.
